interactive pdf only containing buttons turns into form?

i made interactive pdf document export indesign. contains buttons link bookmarks. if open in acrobat dialog pops-up requesting fill out form.

there no form fields in pdf. tried kinds of things , seems randomly appear. when exported parts of document appears , not, there's absolutely no logic. removed buttons , started adding them 1 page @ time , problem occurs random. i'm completelylost here.

button form fields! particular number (any kind of) form fields triggers bar in acrobat. there´s several threads issue in forum, , @ least couple of work-arounds how can avoid notification, use forum search find threads... (or maybe else has bookmarked them?)
