Photoshop creating boxes on layer mask after sharpening


i don't know how explain hope screen shot i've included explains better. have problem photoshop automatically creating strange boxes on layer mask when i'm sharpening image, first high pass, unsharp mask. after i've created stamp visible layer on top of layer mask. don't understand! can me this?

i don't see "layer mask" exept 1 layer, appears full white, meaning opaque. otherwise history panel denotes blending mode change, whole reason - applying filters intensifies invisible artifacts due how interacts underlying layers. chances remnant of sloppy work on 1 of lower layers, not actual layer itself...
